Our patients get official written documentation as follows:
  • Terms and Conditions of Dental Treatments (this present document)
  • Patient data sheet
  • Treatment plan and Quotation
  • Declaration of consent before root canal treatment, implantation, surgery, teeth whitening and general anaesthesia
  • Acknowledgement of receipt and guarantee statement
  • Final invoice
  • Pre and Post treatment instructions

At the first visit, you should fill in a Patient data sheet and sign it. The data sheet contains personal data like name, address, contacts, allergies, sickness and medications, etc. One copy stays with the clinic and you can get a copy by your request.

Before the treatment, we prepare Treatment plan and Quotation

If you accept the Treatment Plan and Quotation, you and we should sign them. You will get a copy and a copy stays with the clinic.

All surgeries carry certain risks of complications. Our dentist will explain the possible anticipated outcomes and risks of your surgery and treatment. Before root canal treatment, surgery, implantation, teeth whitening and anaesthesia the patient has to sign a document called Declaration of consent. One copy stays with the clinic and you can get a copy by your request.

When the treatment completed and you are satisfied with the result, you and we should sign the Acknowledgement of receipt. The documents declare the result of the treatments and the guarantee periods. One copy stays with you, the other with the clinic.

For any payments, the clinic will issue the Final invoice. You should to sign the invoice. One copy stays with you, the other with the clinic.

By the end you will get the After Treatment Instructions what you should follow after the treatment. You should sign it and one copy stays with you, the other with the clinic.
